To support this, we offer a unique service designed primarily for inpatients who have participated in our rehabilitation programs, especially those with neurological conditions or chronic diseases. This complimentary service, unmatched across the country, reflects our dedication to patient-centred care and well-being.

Making the Patient’s home accessible

Returning home after hospitalization can present challenges, especially for patients with mobility issues. To ensure a safe and accessible environment, our team conducts a comprehensive home assessment.

Who is this Service For

This service is specifically tailored for patients recovering from neurological conditions such as stroke, multiple sclerosis, or Parkinson’s disease. It is also designed for individuals managing chronic diseases, including musculoskeletal disorders or long-term disabilities, who require ongoing care and support to maintain their quality of life and independence.

What We Do:

One of our trained representatives will visit the patient’s home to assess the overall accessibility of the living space and identify any structural adjustments or equipment needed to enhance mobility and safety.

Home Assessment Checklist:

  • Entrance: Determine if a ramp is required for wheelchair access.
  • Doorways: Verify if they are wide enough for wheelchairs or mobility aids.
  • Stairs: Assess whether a stairlift or similar solution is needed.
  • Bedroom: Evaluate the necessity of an automated or electric bed.
  • Bathroom: Recommend modifications, such as installing grab handles, non-slip flooring, accessible showers, and adaptive toilets or commodes.


Equipment Recommendations:
Patients may either purchase the required equipment directly or apply for government funding to cover the costs.

Assistance with Medical Equipment:

Our licensed physiotherapists play a key role in identifying and recommending the most suitable medical equipment for each patient’s needs. Depending on the patient’s requirements, we can recommend:

  • Ready-made aids, such as standard wheelchairs or walkers.
  • Custom-made equipment tailored to individual needs, such as specialized prosthetics or orthopedic devices.


What We Provide:

  • Accurate measurements and specifications for equipment to ensure an optimal fit.
  • Guidance on sourcing reliable suppliers.

Assistance with Funding:

For patients seeking government funding for equipment:

  • Applications must be evaluated and signed by the Head of our Physiotherapy Department.
  • Patients will be guided to obtain three quotations from different suppliers. These quotations, along with the funding application, will be submitted to the relevant government authority.


Process Overview:

  3. Submit the application through our team for review and approval.
  4. Wait approximately 15 days for the funding decision.

Our staff is here to guide you through each step of the process to ensure timely submission and approval.

Recommendations for future care:

We believe in continuity of care, even after discharge.

Follow-Up Evaluation:

  • For patients who have completed their rehabilitation program before discharge, our physiotherapist will conduct a follow-up evaluation 15–20 days post-discharge to monitor progress.
  • Based on this evaluation, we may recommend:
    • Home-based exercises tailored to the patient’s condition.
    • Suitable sports activities or physiotherapy techniques to improve mobility and strength.


Continuing Physiotherapy:

  • Patients discharged before completing their rehabilitation program will be encouraged to continue with outpatient physiotherapy sessions to maintain and enhance their recovery
